Dr Patrick Brooke
Patrick is a GP and the Chief Accountable Officer for NHS Solihull Clinical Commissioning Group. Prior to this he was Deputy Director of Commissioning Development for Birmingham and Solihull Primary Care Trusts, Medical Director of Solihull Care Trust and from 1996-2008 a GP in Newbury, Berkshire. He continues to practice as an ‘Out of Hours GP’ in Worcestershire.
Patrick has been working with commissioners and providers to lead a large scale transformation agenda in Solihull that focuses principally on more integrated care for frail elderly and dementia, together with improvements in local Urgent Care provision. Regionally he has led on the Birmingham and Solihull Dementia Strategy.

From 1998-2003 he worked as a clinical assistant in dementia research developing and validating ‘The 6CIT’ as a dementia recognition tool for primary care and also co-developed a local integrated care pathway for people with dementia. Subsequently he co-researched a project to implement this pathway in West Berkshire.
In 2004 he co-developed and researched a primary care based memory service that reduced waiting times and improved service quality through redesign of existing services.
